How
to Become a Care Card Member:
Fill
out and sign a Care Card Membership™ application
with a one-time enrollment fee of only $30. You
may enroll as follows:
Monthly
fee
Individual (adult 18 and older) $35
Couple (2 adults or 1 adult and 1 child) $70
Family (Limit of 4 - $15.00 additional monthly
fee for each additional member) $125
Pay
only $10 for an office visit which can include
if needed -
Up
to three lab tests per visit including urinalysis,
complete blood count (CBC), strep screens, mono
tests One X-ray (one body area per visit) One
EKG
Free blood pressure checks - always
If
additional services are needed, we can provide
them at an additional cost.
Single ($35.00 per month)
Couple ($70 per month)

Family ($125 per month)
What
Is The Eligibility Criteria?
Simply
do not have medical insurance (uninsured) or have
an insurance plan that we do not particpate with.
How
do I make payments?
There
are 2 methods of payment:
1.
Auto deduction for Credit or Debit card or Checking
account.
or
2.
Cash payment for annual membership (1 full year)
which engages an automatic 10% discount
When
do I become eligible to use my Care Card Membership™?
Your membership will become active seven days from
date of application to allow for processing. All
benefits are provided at Complete Care Center in
Grand Blanc.
Will
I receive a membership card?
Each
individual and family member will receive a Care
Card Membership™ card with photo. The photo will
be taken at Complete Care Center at the time of
application or before your membership becomes
active. You will need to bring your membership
card to each office visit.
Are
there any age restrictions?
Services
are available to anyone from the age of six months
or older.
Will
my pre-existing medical conditions be accepted?
Yes.
We will treat only those conditions which can
be treated Complete Care Center. We provide an
extensive, comprehensive range of medical
services with extended hours.
However, patient do not qualify if they have chronic
pain conditions requiring narcotic medications.
Where
can I receive a complete listing of medical
services available with my Care Card
Membership™?
A complete listing of medical services is available
on our website at www.completecarecenter.net
or at our front desk.
How
do I terminate my Care Card Membership™?
Your
membership can be terminated at any time for a
one-time $20 cancellation fee and we will assist
you in accomplishing that. Pre-paid monthly memberships
are non-refundable, but unused monthly portions
of an annual membership will be refunded within
30 days.
If you should no longer meet the criteria (no
longer uninsured) for membership eligibility,
you must notify Complete Care Center immediately
to terminate your membership.
Do
I Need Catastrophic Insurance?
Catastrophic
insurance is insurance that covers you if major
hospitalization occurs. 85% of health care expenses
are for non catastrophic, non hospital health
care. Whether you need it or not is a personal
choice. If you feel you do, we suggest one of
two plans from Aetna. One is preventative and
hospital care 1250 and preventative and hospital
care 3000 (HSA compatible). Why do we suggest
these two: We have had inhouse billing for 15
years. We are familiar with all of the hurdles
and barriors and excuses that insurance companies
create for non payment of benefits. These two
plans are very simple, do not require any complicated
billing on your part or on the part of the physician
for services not hospital related. Both of these
plans are very compatible with the Care Card Membership.
